What is a DBQ? How the Disability-Based Questionnaire Works for VA Disability 

A DBQ is a form that helps Veterans provide medical evidence to support their VA disability claims. It allows a medical professional to document a Veteran’s condition in detail, focusing on the specific information required by the VA to assign a disability rating. 

What Does DBQ Stand For? 

DBQ stands for Disability Benefits Questionnaire. It is designed to simplify the VA disability claims process by giving physicians a structured way to describe a Veteran’s condition, symptoms, and how it impacts daily life. 

How Do I Use a DBQ Form? 

Veterans can have a private doctor or VA physician complete a DBQ. It can then be submitted as evidence to the VA during the claims process. The form is beneficial for both new claims and for appeals where more detailed medical information is required. A variety of DBQ forms are available on the VA website

Who is Allowed to Fill Out a Disability Benefits Questionnaire? 

DBQs can be filled out by licensed medical professionals. This includes both VA and private doctors. However, having a private physician complete a DBQ can be advantageous, especially if the Veteran faces delays in getting VA appointments. 

Do I Need a C&P Exam for My DBQ? 

A DBQ can sometimes replace a Compensation & Pension (C&P) exam if it contains enough detailed information for the VA to make a decision on the claim. However, in some cases, the VA may still require a C&P exam even with a completed DBQ.
