Entitlement to earlier effective dates for service connection related to intervertebral disc syndrome, right lower extremity radiculopathy, and scar status post L5/S1 hemilaminotomy and microdiscectomy is granted due to clear and unmistakable errors, resulting in evaluations of 40 percent, 20 percent, and 0 percent, respectively, effective July 19, 2011, and December 22, 2011. However, requests for higher evaluations for right lower extremity radiculopathy and scar status are denied.
